Small incision lenticule extraction, better known as SMILE, is a one-step laser surgery that corrects near-sightedness and astigmatism, and its price is one of the first things Portland patients ask about. Here is what SMILE actually costs, what goes into that number, and how patients at Tersigni Vision pay for it.
How much does SMILE eye surgery cost in Portland?
The average cost of SMILE eye surgery in Portland is $2,500 to $4,000 per eye. That is a wide range for a single procedure, and the spread comes down to real differences between practices, including the:
- Generation of laser platform used
- Depth of diagnostic testing, including corneal topography
- Surgeon’s fellowship training and refractive experience
- Strength of your eyeglass prescription and the degree of correction needed
- Number of LASIK alternatives the practice actually offers
- Time the surgeon spends with each individual patient
- Level of follow-up care included after surgery
Because SMILE is only one of the procedures we perform at Tersigni Vision, we give you an exact price at your consultation, once we know whether SMILE is genuinely the best match for your eyes.

Why does the price of SMILE vary so much between practices?
The Portland vision correction market can be confusing. Providers advertise rotating specials and headline prices that often climb once you are through the door and sitting in the exam chair.
That is not how we do things at Tersigni Vision. The value of a SMILE procedure is not in the 23 seconds the laser is running. It is in the diagnostic work that determined SMILE was right for you, the experience of the surgeon performing it, and the care you receive while your eyes are healing.
A practice that offers only one or two procedures has to fit every patient into what it already owns. A practice that offers the full portfolio of modern options can afford to tell you that SMILE is not your best choice, because it has something better to offer instead.
What is included in the cost of SMILE at Tersigni Vision?
Your price covers considerably more than surgery day. Every patient at Tersigni Vision begins with our Advanced Ocular Analysis, a 90-minute doctor’s visit that uses an extensive array of diagnostic devices to measure and scan the entire eye in three dimensions, from the cornea in front to the retina in back.
Corneal topography is a central part of that workup. Mapping the curvature and thickness of your cornea is what tells us whether SMILE is safe and appropriate for you, or whether LASIK, PRK, an EVO ICL implant, or a lens-based procedure would serve you better. The exam also includes an in-person evaluation with Dr. Tersigni himself, not a sales consultant.
Our procedure pricing ranges from $2,200 up to $7,200 per eye. SMILE sits within that range, and we will always be transparent about where, and never recommend a procedure you do not need.
Does SMILE cost more than LASIK?
SMILE and LASIK are priced closely at most practices, and SMILE is often slightly higher because of the specific laser platform required to perform it. Both procedures deliver comparable visual acuity when treating near-sightedness and astigmatism, so the more useful question is not which one costs less, but which one your eyes are actually built for.
Many of our SMILE patients came to us after being told they were not LASIK candidates, whether because of thin or irregular corneas, a tendency toward dry eye, or a job or sport that carries a risk of direct contact to the eye. For those patients, a lower LASIK quote elsewhere was never a real option to begin with.

Is SMILE a newer procedure than LASIK?
Yes. SMILE has been performed internationally since 2011 and became available in the United States in 2016, while LASIK has been in use domestically since the 1990s. That newer laser technology is part of why SMILE pricing tends to sit at the upper end of the laser surgery range in ophthalmology practices that offer it.
Newer does not automatically mean better for every patient. It means one more option on the table, which is the point of evaluating all of them before you commit to a price.
Come see what you’re missing.
Does insurance cover SMILE eye surgery?
No. Insurance companies classify SMILE, LASIK, and every other refractive procedure as elective, even though the results change everyday life for most patients. You should plan on SMILE being an out-of-pocket expense, which is exactly why we offer several ways to make it manageable.
How can I pay for SMILE eye surgery?
Our goal at Tersigni Vision is to make vision correction fit any budget. Our most popular financing options are:
- Zero down payment with 0% interest for 24 months
- Zero down payment with low monthly payments at a fixed interest rate for 60 months
With financing, laser vision correction can run as low as $140 per month. Our staff can walk you through the details and pre-qualification at your visit.
We also accept:
- Cash or check
- Debit or credit card
- HSA (Health Savings Account)
- FSA (Flexible Spending Account)
- HRA (Health Reimbursement Account)
- MSA (Medical Savings Account)
Most employers that offer an FSA or HSA allow those funds to be applied to vision correction procedures, so it is worth checking your plan before open enrollment closes.
Is SMILE eye surgery tax deductible?
Yes. The IRS treats vision correction surgery as a qualified, tax-deductible medical expense. Whether you personally benefit depends on your overall return, so confirm the details with your tax advisor.
Is SMILE worth the cost?
The average person spends more than $25,000 over a lifetime on corrective lens purchases, solution, and routine exams. That is renting your vision from the eyewear industry, and those temporary fixes never actually correct it.
A patient who has vision correction at 18 is on track to save more than $40,000 in vision-related costs by the time reading glasses enter the picture in their 40s. And that math leaves out everything that does not show up on a receipt, like packing enough contacts for a trip, replacing lost glasses, or dealing with a contact lens-related infection.
With SMILE, you own your vision instead of renting it. Use our affordability calculator to see what the trade looks like for you.
What are the hidden costs of cheap laser eye surgery?
A price well below the Portland average usually means something was cut to get there. Sometimes it is the technology. Sometimes it is the diagnostic workup. Sometimes it is the surgeon, in the form of a visiting doctor you meet minutes before your procedure and never see again while you are healing.
Practices that advertise free consultations have to recover that expense somewhere, which often shows up as a shorter exam, less time with the surgeon, and more pressure to say yes on the spot. Before you commit to a low advertised price, ask directly what was reduced to make it possible, and whether the same surgeon will be handling your follow-up care.
You only have one set of eyes. Price belongs in the decision, but it should sit alongside training, technology, and outcomes rather than ahead of them.
How do I get an exact price for SMILE?
The only way to know what your procedure will cost is to find out which procedure you need. At Tersigni Vision we start with the Advanced Ocular Analysis, review your results with you in person, and quote you a firm price for the option that fits your eyes and the way you want to live.
